Preparation
Gather All Required Equipment:
- New sterile tracheostomy tube (same size or as prescribed).
- Sterile gloves.
- Suction machine with catheters.
- Lubricating jelly (water-based).
- Sterile saline or water.
- Gauze pads and dressing.
- Ties or Velcro strap for securing the tube.
- Scissors.
- Rolled towel to place under the shoulder blades to extend neck.
- Ambu bag with oxygen source or self-inflating resuscitation bag.
- Pulse oximeter monitor and probe.

Remove Old tracheostomy Tube

- Hold the dirty tracheostomy tube in place as you cut the ties.
- Take a deep breath before taking out the tracheostomy tube.

- Secure the tracheostomy ties.
Insert New Tube
- Our Nurse picked up a new, clean tracheostomy tube with the obturator in place.
- Insert the new, clean tracheostomy tube from a side angle with the obturator in place for smooth insertion, rotating into the correct position while pushing it further into the neck opening (right handed rotate counter-clockwise, left handed rotate clockwise).
- If there is any resistance, Our nurse does not keep pushing. Instead, remove the tracheostomy tube, look closely at the tube and neck opening, and try again.
- A cuffed tube will have mild resistance, which is normal.
- Once the tracheostomy tube is in place, our nurse holds the new tracheostomy tube in place with two fingers on the faceplate, and removes the obturator.
- Airflow will be blocked by the obturator while it is still inside the tracheostomy tube.
- Secure the new clean tracheostomy collar.
- Sit the patient up to check how snug the tracheostomy collar is. You should be able to fit one pinky finger between the tracheostomy collar and the skin on the right side, left side, and back of the neck.
